Periodontal Maintenance Add-On — $125/year
Need more frequent care? This add-on includes a third periodontal maintenance cleaning each year (instead of the standard two). Any additional cleanings receive a 15% discount.
Good to Know
- The annual fee is due at the time of enrollment (tax included).
- Covers one individual for 12 months from your signup date.
- Benefits must be used within the 12-month period — no refunds or extensions.
- No discounts on products such as toothbrushes or prescription toothpaste.
- Cannot be combined with dental insurance or any other savings plan.
