Whether it’s your first time visiting our dental office or you’re looking for a new dental home, our easily accessible patient forms will make your visit a breeze. These forms give the Summit Dental team relevant information and background about your dental health that will help us determine your needs and accurately assess your oral health. We are offering these electronically to reduce your contact in-office.

For those who are joining our family of patients, it might seem out of the ordinary to be scheduled for a 90-minute new patient visit with the dentist and hygienist. This time is reserved especially for you to ensure that you can receive a thorough dental examination and consultation with Dr. Nelson or Dr. Ford. They look forward to getting to know you and your family, and to talk with you about your oral health and expectations for your smile. Our goal is not only to take care of our patients’ dental needs but also to get to know each person by name and treat them like we’d want our own family to be treated. Will my appointment be at the scheduled time?

We strive to run on time so you won’t be kept waiting, and we ask you to arrive for your appointments on time as well. We understand that you are busy, and your time is valuable to us. Dr. Nelson and her staff only deviate from her schedule in the event of dental emergencies that need to be taken care of immediately.

Do you offer preventative appointments and treatment?

Once we have decided on a treatment solution for your needs and wants, we will do everything we can to serve you most appropriately. We will always work with your insurance on your behalf to maximize your insurance coverage. It’s just one less thing you have to worry about. There is little or no cost for preventive care for most insured patients and our Summit Smiles Program offers predictable value. It’s part of our commitment to preventing uncomfortable and costly dental work – by helping you resolve issues before they become major problems.

What if I need to change an appointment?

We understand that life happens. Should you need to change or reschedule a reserved appointment time, please call at least 48 hours in advance so that another patient can be seen sooner. We reserve the right to charge a fee for appointments canceled within 48 hours.

What is your privacy policy?

We take your privacy seriously. You can receive our privacy policy in the office or at this link.
